The photo should be 2 inches by 2 inches (51mm x 51mm).
The photo should be close of head and top of shoulders so that face takes up 70%-80% of the photo.
The skin tone should be natural and should have appropriate brightness and contrast.
The image should be straight looking, centered with neutral expression.
The eyes must be open and hair should not cover the face.
If glasses are worn then should be clear and thin framed and should not have flash reflection or cover the eyes.
Head coverings, hair, head-dress or facial ornaments should not cover the face.
The photograph must be taken in a colored attire, for example, a medium blue shirt etc.
The attire should not be patterned or textured or pure white.
The photo must have WHITE background.
There must be no other people or object in the background of the photo.
The lighting must be uniform and should not have any shadows on the face or behind.
Red eye photos will not be permissible.
Photo should not be older than 6 months.
Photo should not be folded or have any crease or line on it
US Passport and Visa Photos Requirements
The Photo must be In color

Taken within the last 6 months to reflect your current appearance
Taken in front of a plain white or off-white background
Taken in full-face view directly facing the camera
With a neutral facial expression and both eyes open
Taken in clothing that you normally wear on a daily basis
Uniforms should not be worn in your photo, except religious clothing that is worn daily.
Do not wear a hat or head covering that obscures the hair or hairline, unless worn daily for a religious purpose. Your full face must be visible, and the head covering must not cast any shadows on your face.
Headphones, wireless hands-free devices, or similar items are not acceptable in your photo.
Effective November 1, 2016, eyeglasses are no longer allowed in new visa photos, except in rare circumstances when eyeglasses cannot be removed for medical reasons; e.g., the applicant has recently had ocular surgery and the eyeglasses are necessary to protect the applicant eyes. A medical statement signed by a medical professional or health practitioner must be provided in these cases.
If the eyeglasses are accepted for medical reasons:
The frames of the eyeglasses must not cover the eye(s).
There must not be glare on eyeglasses that obscures the eye(s).
There must not be shadows or refraction from the eyeglasses that obscures the eye(s).
If you normally wear a hearing device or similar articles, they may be worn in your photo.
